Youth Development Specialist - (Weekends)

 Job Purpose:
To provide services that will help improve the social and psychological functioning of young males in a group home setting.

We are looking for Energetic individuals who can provide care for up to 5 males, ages 12-18, in a 24-hour group home setting. Ability to work in a fast-paced environment, while providing quality care is essential. The ideal candidate will have strength in working with at-risk youth, have excellent written and verbal communication skills and 2 years of experience working with youth. Essential Duties and/or Responsibilities:
Youth Development Specialist will supervise and care for adolescences in the group home. Youth Development Specialist are responsible for maintaining a therapeutic environment for the residents while running the daily schedule and routine of the home. Youth Development Specialist work both one-on-one with youth in the home, as well as run various group recreational activities. Youth Development Specialist will interact with residents while providing a positive, safe and caring environment. Youth Specialist will assist program participants as needed. Provide routine Case Management for residents, maintain accurate documentation as required. Supervise residents during visitations, educational and recreational activities. Youth Specialist will consult with parents, teachers, case workers and other school personnel to determine causes of problems such as truancy and misbehavior, and to implement solutions. Youth Specialist also counsel residents whose behavior, school progress, or mental or physical impairment indicate a need for assistance.

Youth Development Specialist will also help identify building maintenance issues and submit work orders to the Revive Youth & Family Center’s program Director. Monitor building cleanliness, performing housekeeping duties when necessary to ensure the highest standards are maintained daily. Keep inventory of cleaning supplies, prepare meals for residents. Monitor food sanitation. Maintain safety and security of the house. Complete other duties as assigned.

Case Manager

Job Purpose:
Primary responsibilities are to provide clinical supportive services to residents while assisting Revive Youth & Family Center in expediting permanence for the residents in our care. Revive Youth & Family Center’s Social Worker/Case Manager will provide counseling for youth at Revive Youth & Family Center. The Social Worker/Case Manager will be responsible for assessment, service plan development, ongoing care coordination, quality assurance and ongoing monitoring of services rendered to the residents of Revive Youth & Family Center. **This position allows for a flexible schedule which may include on-call responsibilities and or working actual shifts.

Qualifications:
• Preferred BSW from a 4-year college.

• Experience with group foster care home preferred but not required.

• Demonstrated ability to take direction from supervisor and work somewhat independently.

• An understanding of Department of Health & Family Services Licensing and the regulations controlling Foster Care Children and Families.

• Demonstrated ability to maintain good relationships with all staff and residents.

• Possess a valid Wisconsin Driver’s License.

• Belief in the purpose and program of Revive Youth & Family Services, LLC

General Responsibility:
• Provide case management by being responsible for maintaining the compliant contents of the resident’s files, creating and ensuring implementation of the treatment plan, monitoring the resident’s activities through visits and interacting.

• Work Closely with the residential program director in actively planning for a residents permanency and discharge from the Revive Youth & Family Center’s home.

• Make sure all residents are up to date on all medical and dental needs.

• Solve practical problems and deal with a variety of unpredictable situations where only limited standardization exists.

• Provide support and education to the parents of residents of Revive Youth & Family Center.

• Submit reports to the DMCPS, court and Department of Health & Family Services as required.

• Provide training to potential and current resource families and staff.

• Coordinate and/or attend treatment trainings, team meetings, and other case specific meetings.

• Advocate for the services needed for residents.

• Help update and prepare paperwork.

• Work cooperatively with staff and volunteers.

• Participate in group processes and activities such as staff meetings, trainings, and conferences.

• Complete special projects as required.

• Perform any additional tasks as assigned by the Residential Program Director.
